Health Assessment for Students
Health Assessment Waiver 2021-22
Following the BCCDC’s COVID-19 Public Health Guidance for K-12 School Settings, all parents, guardians, and/or caregivers have the responsibility to conduct a Daily Health Assessment of their child student every day before sending them to school.
Parents/guardians will be required to self-assess their child for key symptoms of illness daily, before sending them to school. These symptoms include: Fever (Body temperature > 38°C), Chills, Cough or worsening of chronic cough, Difficulty breathing, or Loss of sense of smell or taste. As a reminder, if your child is experiencing any of the above symptoms they should stay home for 24 hours from when the symptom(s) started. If the symptom(s) improve, they may return to school when they feel well enough. If the symptom(s) persists or worsens, seek a health assessment by calling 8-1-1.

Completion of this waiver confirms that as the primary caregiver, you will conduct a daily health assessment of your child BEFORE sending them to school. Families will only need to complete this waiver once per school year.
